A sales issue probably doesn't start in sales.
A marketing issue probably doesn't start in marketing.
An operations issue probably doesn't start in operations. You get it...

The function is often where the problem becomes visible but not where it starts from.

When I dig deeper, the root cause usually comes down to one of three things, in this order:

โ€ข Culture
โ€ข Process

For the entrepreneurs actively running companies, which has been the hardest to fix: people, culture, or process?
